Cardiopulmonary Rehabilitation Program

People of all ages and heart diagnoses can benefit from regular exercise; however, it is important to exercise safely if you suffer from heart disease. Our cardiac rehab program provides a place for monitored exercise; social support and access to experts who will help you successfully manage your heart health.

The cardiac rehab team is lead by Registered Nurses, an exercise physiologist and exercise technicians who work with a medical director, a registered dietitian, and your cardiologist to provide the best care for you as an individual.

Exercise Program

The Heart and Vascular Center’s cardiopulmonary rehabilitation program empowers patients to take action to prevent future cardiovascular problems from happening. Services include medication review, one-on-one dietary counseling, goal setting, exercise evaluation, a mid assessment with the medical director, and support

Your exercise prescription is determined based on your current physical status. A low-intensity exercise test, 6 minute walk) will be completed during your first visit to give us data to tailor the workout to you and your goals. Equipment such as stationary bicycles, treadmills, resistance training machines, free weights and resistance bands offer a full body workout to safely, and efficiently improve your heart’s strength.

During your sessions, professionals will monitor the electrical rhythms of your heart using our cardiac telemetry system, assess responses to exercise such as blood pressure and heart rate in order to properly progress your personal exercise prescription. For those patients with diabetes, we monitor your blood sugar before and after exercise. By using these values, our team of experts will gradually increase the intensity and duration of your program in order to improve your condition.

The team members will communicate openly with your referring cardiologist and are equipped to handle any emergent situations. Our program follows the strict certification guidelines of the American Association of Cardiovascular and Pulmonary Rehabilitation (AACVPR).

In general, you will be exercising in a group setting alongside other like-minded individuals with similar goals (maximum number of participants per class is 16).

The camaraderie that develops among participants during the 12-week program helps each individual maintain the lifestyle changes over the long term. Participants exercise three times a week in a monitored setting, to ensure each participant’s workout is both effective and safe.

Education Classes

Patients will attend an education class lead by a registered nurse, respiratory therapist, exercise physiologist and a dietitian.  Topics are discussed in a group setting to ensure that you have the tools and knowledge you need to live a healthy lifestyle after a heart event.

Our team of experts can help you gain valuable insight into how your heart and lungs works, and more importantly, how you can help it work better. Our education topics include stress management, heart healthy nutrition, heart medications, cholesterol management, high blood pressure, and other topics to help reduce the risk factors for heart disease.

Phase II - Monday, Wednesday & Friday
8:00 a.m., 9:30 a.m., 11:00 a.m.
2 p.m. and 3:30 p.m.

Phase III - Tuesday & Thursday
8 a.m. until closing at 4 p.m.

Transitional Care Clinic (TCC)

The TCC serves congestive heart failure patients, and those suffering from COPD or pneumonia, who are over the age of 55, and have visited the hospital more than twice in six months, as well as patients who do not have support at home or an existing relationship with a primary care physician. At the TCC, staff can review patient history, help answer questions, steer patients to a primary care physician, order lab work and create future care plans such as a schedule for cardiac rehab visits. Call to learn more: 301-877-4661.
